How weight loss and blood sugar affect heart-protective HDL cholesterol

This research is studying a special protein called paraoxonase that sits on HDL cholesterol — the 'good' cholesterol that protects your heart. Researchers want to understand why this protective protein sometimes doesn't work well in people with diabetes or obesity, even when HDL levels look normal on blood tests. You'll be asked to donate blood samples so scientists can measure how this protein functions in different groups: people with Type 1 or Type 2 diabetes, people having weight-loss surgery, and healthy volunteers.

People with diabetes and obesity have a much higher risk of heart disease, but doctors have noticed that normal cholesterol numbers don't always mean the heart is actually protected. This study exists to understand whether the problem is the amount of cholesterol or how well it actually works — and whether weight loss or managing blood sugar can restore the protective function.
