This study doesn't follow the usual testing phases — it may be an observational study or a different type of research.
This trial is testing a new way to care for people who have chronic lung disease (COPD) or heart failure and also live with multiple other health conditions like diabetes, high blood pressure, or kidney disease. Instead of treating each disease separately, doctors will review all of a patient's medications and conditions together, then create a personalized treatment plan. Participants will also join a 6-month moderate-intensity exercise program, and researchers will track their health over 3 years to see if this comprehensive approach reduces flare-ups, hospitalizations, and improves quality of life.
Many people with chronic lung disease or heart failure also have other chronic conditions, which can make their main disease worse and harder to control. This trial exists to find out whether treating all of someone's conditions as an interconnected system—rather than in isolation—can better prevent dangerous flare-ups and hospitalizations and help people live longer.

If you are accepted into this study, you'll be part of one of three groups: people with chronic lung disease, people with heart failure, or healthy volunteers (for comparison). Over 36 months, you'll have visits at the start and then at 12, 24, and 36 months. At each visit, your doctor will review and optimize all your medications, blood tests will check inflammation markers and other health indicators, and you'll complete questionnaires about your symptoms and quality of life. If you're in the treatment groups, you'll also participate in a 24-week moderate-intensity exercise program tailored to your abilities.
