If you're selected, you'll be randomly assigned to either the CORE program or standard care group. The CORE group will receive a personalized exercise program (both in-person at the study center and at home via video), help managing heart disease risk factors, and behavioral support from professionals and other participants for 6 months, then ongoing behavioral support through month 24. Everyone wears a wrist monitor to track physical activity and heart rate throughout the 2-year study period. Standard care participants receive usual medical care and the same activity monitor.
