Phone Support From a Nurse to Help Black Men Control Type 2 Diabetes

This study is testing a new way to help Black men who have been recently released from prison manage their type 2 diabetes. Instead of seeing a nurse only at a clinic, you would receive phone calls from a trained nurse who helps you with diabetes skills, finding healthcare services, and meeting basic needs like food and housing. The goal is to see if this personalized phone support helps people control their blood sugar better and feel healthier.

Black men who have experienced incarceration often face unique challenges in managing diabetes — including trouble accessing healthcare, unstable housing, and difficulty getting resources they need. This study exists to test whether a nurse who understands these specific challenges can provide better, more practical support than standard diabetes education alone.
