Better Hospital Discharge Support for Liver Cirrhosis with Fluid Buildup

This pilot study is testing a new discharge support program for people with cirrhosis who have excessive fluid buildup (swelling). Instead of standard discharge instructions alone, the program provides close monitoring through weekly phone calls, a digital scale to track weight at home, educational materials, and an early appointment with a liver specialist. Researchers want to see if this extra support helps prevent future hospitalizations, kidney problems, and improves survival compared to standard care.

People with cirrhosis often develop dangerous fluid buildup and need hospital care, but after discharge they may not get enough follow-up support. This trial exists to find out whether a structured monitoring program can catch problems early and keep patients healthier at home.
