If you're enrolled, you'll be randomly assigned to either receive help from a care champion or to follow standard care. If assigned to the care champion group, you'll meet your champion before leaving the hospital, receive education about your cholesterol goals and medications, and stay in contact with them over the next several months as you transition to outpatient care. All participants will have their cholesterol levels checked at baseline, 6 months, and 8 months, and will answer some questions about their health and care. The entire study lasts about 8 months.
