Better care for people with HIV and other chronic health conditions

This study is looking at a new way to organize care for people living with HIV who also have other chronic health conditions like high blood pressure, diabetes, or heart disease. Instead of making you go to multiple clinics or appointments, the study is testing a flexible care model that tailors services to your individual needs and what's available in your local area. The goal is to give you better, more coordinated care while reducing the burden on clinics.

People living with HIV who take HIV medications effectively can live as long as anyone else, but they face twice as many complications from other chronic diseases. Right now, managing both HIV and these other conditions can be complicated—clinics may lack resources, or you may need to see multiple specialists. This study exists to find a better way to deliver care that works for both patients and healthcare systems.
