Supporting heart failure patients and caregivers at home in rural areas

This study is testing a new support program called HF-FamPALhomeCARE designed to help people with advanced heart failure and their family caregivers. Half of participants will receive regular nurse-led visits and phone calls focused on managing heart failure at home, along with discussions about end-of-life care planning. The study will track both patients and caregivers over one year to see if this extra support improves their quality of life, mood, and sense of preparedness compared to standard care alone.

Advanced heart failure is a serious condition that affects not only patients but also their family caregivers, especially in rural areas where access to specialized support can be limited. This trial is testing whether regular, personalized support from nurses can help people manage their condition better at home and reduce stress for both patients and those caring for them.
