Online support group to help heart failure patients feel less isolated

This study is testing a new approach called SOCIAL_HF — a 6-week online support program designed specifically for people with heart failure. The program uses WeChat (a messaging app) to connect patients with each other through weekly video meetings, daily check-ins, and shared health goals. The hope is that this sense of connection and community will help reduce the loneliness and isolation that often comes with heart failure, while also improving how well patients manage their condition.

Many people with heart failure become socially isolated because of symptoms like shortness of breath and fatigue, which makes it harder to stay connected to others and take care of themselves. Research shows that this isolation can actually make heart failure worse and lower survival rates. This trial exists because there aren't many proven ways to help patients reconnect socially and break this harmful cycle.
